The doctor made an excellent first impression on most people who encountered him. His youth made him stand out among his older colleagues, as did his fashion sense. A carnation was often tucked neatly into the button hole of his pinstripe suit. To the public, he was celebrated as a talented surgeon. Those who knew better said nothing. They were too embedded in the culture of doctor knows best.

When I was in hospital, which was in 1984, the nurses called him a butcher. One nurse said she wouldn't let him pierce her ears, and they said it wasn't appalling, and they said I was the most desperately ill person they'd ever seen survive, and that I should sue because it was happening in every single one of his cases.
